Policies

Please find some of our office policies below. They are designed to make our patients feel at ease and provide you with the care that you deserve. If there is anything we can do to make your visit more pleasant, please notify us.

New patients should arrive 15 minutes prior to their appointment time so that our staff may get your information registered within our system. Please bring the patient forms to your appointment. We ask that you take the time to print and complete all of these forms and bring them with you to your appointment. If you have not done so, please plan to arrive 30 minutes prior to your appointment.

We also ask that you bring your current driver’s license and insurance card(s) with you. This will help to ensure your insurance claim(s) are filed correctly. Please bring a list of the current medications (including vitamins and dietary and herbal supplements) you are taking as well as any previous diagnostic tests.

No Show Appointments/Surgeries: The patient will be billed a fee for not canceling an office visit or surgery in advance. This fee will be billed directly to the patient.

Prescriptions

Patients are requested to fax the office a prescription refill request 24-48 hours in advance. Refills will not be handled after hours or on weekends.

Phones are answered Monday through Friday, 8 a.m. to 5 p.m.
